As a result of this A/E review, the A/E agrees that there are no changes to his contract work
which may result in a fee adjustment except as noted below by the A/E:

The Contracting Officer (CO) will process a change order or supplemental agreement to the
A/E's contract for the noted items which the CO agrees are changes to the contract work. The
CO will advise the A/E as to how to proceed on the noted items which the CO disagrees are
changes to the contract work. The A/E is reminded that the Department of Veterans Affairs
will not compensate him for extra work accomplished without the contractually required
notification to the CO and the issuance of a change order or supplemental agreement.
This agreement is to be completed and signed by the A/E and returned to the CO within five
work days after
(date)*. If this form is not returned within five work days, the CO will
presume there were no changes in the A/E's contract work resulting from this review.
