VA hires the best A/E firms in the country and yet we experience repeated
errors and omissions on every project. These are not new or unusual items,
but they consistently get past A/E and VA quality assurance reviews and result
in construction change ord ers. Please review your project documents and
make sure you do not make these mistakes.
Site and Landscape, Design Development Phase :
o Identify structures for demolition, especially where utilities are located; existing sheds and
other minor structures are not properly identified for demolition;
o Provide fences or other protective means around ponds, electrical transformers, and other
equipment to insure patient safety;
o Identify existing deteriorated side walks, near new construction, for demolition and
replacement, if they present a potential hazard; and
o Changes in elevations are required to site features due to inadequate utility surveys; and
often during construction, existing utility elevations and locations cause change orders for
various site features.
