Provide one per Head Nurse FTE position authorized. (OFA01 for standard furniture
or OFA02 for systems furniture)
Location to be adjacent to the Nurse Station.
4. Office, Ward Clerk (OFA03) ...................................................... 80 NSF (7.5 NSM)
Provide one per Ward Clerk FTE position authorized.
The location of the Ward Clerk Office may be in a separate room and interconnected
with the Nurse Station for performance of Medical Administration duties.
5. Office, Physician / Consult (OFD01) .................................... 120 NSF (11.2 NSM)
Provide one per Physician / Consult FTE position authorized.
6. Office, Physician's Assistant /
Nurse Clinician (OFD01) ....................................................... 120 NSF (11.2 NSM)
Provide one per Physician's Assistant / Nurse Clinician FTE position authorized.
7. Office, Psychologist (OFDC1) ............................................. 120 NSF (11.2 NSM)
Provide one per Psychologist FTE position authorized.

Unit, if provided, and when office space is not provided for each psychologist,
psychiatrist or chaplain FTE position authorized assigned to the Substance Abuse
Unit.

Provide one per Social Worker FTE position authorized and if office space is not
provided in Social Work Service (Chapter 282).
12. Office, Dietician (OFDC1)...................................................... 120 NSF (11.2 NSM)
Provide one per Dietician FTE position authorized and if office space is not provided
in Nutrition and Food Service (Chapter 224).
13. Office, Nursing Supervisor (OFA01 / OFA02) ..................... 120 NSF (11.2 NSM)
Provide one per every four Mental Health and Behavioral Patient Care Units and if
an office is not provided in Nursing Service Administration (Chapter 254). (OFA01
for standard furniture or OFA02 for systems furniture)
