I.RespectandNondiscrimination
Youwillbetreatedwithdignity,compassionandrespectasanindividual.Yourprivacywillbeprotected.Youwillreceive

Asaninpatientorlongtermcareresidentyoumaywearyourownclothesandkeeppersonalitems.Thisdepends
onyourmedicalcondition.

Asaninpatientorlong-termcareresident,youhavetherighttosocialinteractionandregularexercise.Youwillhavethe
opportunityforreligiousworshipandspiritualsupport.Youmaydecidewhetherornottoparticipateintheseactivities.
YoumaydecidewhetherornottoperformtasksinorfortheMedicalCenter.
Asaninpatientorlong-termcareresident,youhavetherighttocommunicatefreelyandprivately.Youmayhaveorrefuse
visitors.Youwillhaveaccesstopublictelephones.Youmayparticipateincivicrights.
Asalong-termcareresident,youcanorganizeandtakepartinresidentgroupsinthefacility.Yourfamilyalsocan

meetwiththefamiliesofotherresidents.
Inordertoprovideasafetreatmentenvironmentforallpatientsandstaff,youareaskedtorespectotherpatientsand
staffandtofollowthefacility'srules.Avoidunsafeactsthatplaceothersatriskforaccidentsorinjuries.Please
immediatelyreportanyconditionyoubelievetobeunsafe.
II.InformationDisclosureandConfidentiality

Youwillbegiven informationaboutthehealthbenefitsthatyoucanreceive.Theinformationwillbeprovidedin a
wayyoucanunderstand.
Youwillreceive informationaboutthecostsofyourcare,ifany,beforeyouaretreated.Youareresponsibleforpaying
foryourportionofthecostsassociatedwithyourcare.
Yourmedicalrecordwillbekeptconfidential.Informationaboutyouwillnotbereleasedwithoutyourconsent
unlessauthorizedbylaw(forexample,Statepublichealthreporting).Youhavetherighttoinformationinyour
medicalrecordandmayrequestacopyofyourrecords.Thiswillbeprovidedexceptinraresituationswhere
Violators are
yourVAphysicianfeelstheinformationwillbeharmfultoyou.Inthatsituation,youhavetherighttohavethis
discussedwithyoubyyourVAprovider.
Youwillbeinformedofalloutcomesofcare,includinganyinjuriescausedbyyourmedicalcare.Youwillbeinformed
abouthowtorequestcompensationforinjuries.
III.ParticipationinTreatmentDecisions

You,andanypersonsyouchoose,willbeinvolvedinalldecisionsaboutyourcare.Youwillbegiveninformation
youcanunderstandaboutthebenefitsandrisksoftreatment.Youwillbegivenotheroptions.Youcanagreeto
orrefusetreatment.Refusingtreatmentwillnotaffectyourrightstofuturecarebutyouhavetheresponsibility
tounderstandthepossibleresultstoyourhealth.Ifyoubelieveyoucannotfollowthetreatmentplan,youhavea
responsibilitytonotifythetreatmentteam.
Asaninpatientorlong-termcareresident,youwillbeprovidedanytransportationnecessaryforyourtreatmentplan.
Youwillbegiveninwriting,thenameandprofessionaltitleoftheproviderinchargeofyourcare.Asapartnerinthe

Youhavetherighttohaveyourpainassessedandtoreceivetreatmenttomanageyourpain.Youandyourtreatment
expense.

Youwillbeincludedinresolvinganyethicalissuesaboutyourcare.YoumayconsultwiththeMedicalCenter'sEthics
Committeeand/orotherstaffknowledgeableabouthealthcareethics.

IV.Complaints
Youareencouragedandexpectedtoseekhelpfromyourtreatmentteamand/orapatientadvocateifyouhaveproblems

complainverballyorinwriting,withoutfearofretaliation.
