VA hires the best A/E firms in the country and yet we experience repeated
errors and omissions on every project. These are not new or unusual
items, but they consistently get past A/E and VA quality assurance reviews
and result in construction change ord ers. Please review your project
documents and make sure you do not make these mistakes.
Sanitary, Schematics Phase :
o Do not depend on record drawings for accurate locations of structures, trees,
topography, and utilities. During a site visit of proposed construction, verify location of
all visual sanitary, stormwater, steam, and electrical structures, other visual utility
elements (fire hydrants, fire department connections, post indicator valves, etc.), major
trees, pavement areas, and drainage areas that will be affected by proposed building
and utility construction. If necessary, provide a number of test holes to locate
underground utility lines. Have VAMC verify invert elevation of manholes that will
connect with new work.
o There have been connections to undercapacity, abandoned, or poorly operating
underground piping and the relocation of non-operating fire hydrants. Discuss with the
VAMC/VBA/NCS engineering and maintenance staff the condition and capacities of
underground utilities, fire hydrants, etc. that will be affected by this project and the
possible need for replacement.
